Every plan includes
(cyanocobalamin)
Vitamin B12 is needed to make red blood cells, protect nerves and run the chemistry that turns food into energy. Absorbing it from food depends on stomach acid and a protein called intrinsic factor, both of which fall with age, acid-reducing medication, metformin, and gut conditions. An injection delivers B12 straight into the muscle, so none of that matters. Cyanocobalamin is the most studied and most stable form.
1 in 6
adults over 60 have low or borderline B12, according to the clinical review in the New England Journal of Medicine
B12 injections are well established for deficiency. For people with normal levels, extra B12 does not add energy; your physician checks your level first.

Day 1
A US-licensed physician reviews your symptoms, history and, when needed, your B12 level, and chooses the injection and schedule. The kit includes everything you need.
Week 1
A small intramuscular or subcutaneous injection, weekly to start. It takes under a minute; most people do it at home without trouble.

Weeks 2–4
If your B12 was low, energy, focus and tingling usually start improving here as stores refill.

Months 2–3
Your physician checks in, rechecks a level if needed, and stretches the interval or adjusts the blend to match your plan.

Beyond
Injections support the plan; food, movement and sleep do the work. Your physician helps you decide how long to continue.

Important safety information and common side effects
Vitamin B12 and lipotropic injections are generally well tolerated. The most common effects are mild: soreness, redness or a small bruise at the injection site, and occasionally mild nausea, headache or a fishy body odor from choline in larger doses. B12 is water-soluble and excess is excreted. Rarely, injectable B12 can cause an allergic reaction, and it can lower potassium in the first days of correcting a severe deficiency. Not for people with cobalt allergy or Leber's disease (B12) or with a known allergy to any component. Lipotropic blends are not a substitute for a weight-loss plan and are not prescribed during pregnancy or breastfeeding without a specific reason. Stop and seek care for hives, swelling of the face or lips or trouble breathing. Tell your physician about every medication you take, especially metformin, acid reducers and blood thinners.
Vitamin B12 is well established for correcting deficiency. The question of oral versus injected B12 has been tested in only a few small trials.
The Cochrane review updated in 2018 found three randomized trials with 153 people with deficiency, comparing oral and intramuscular B12. The trials were too few and too varied for meta-analysis, and the authors call for larger, better-blinded trials.
Cochrane Database Syst Rev, Oral vitamin B12 versus intramuscular vitamin B12 for vitamin B12 deficiency (CD004655.pub3)The 2005 version of the same review included two small trials, followed for three to four months.
Cochrane Database Syst Rev (CD004655.pub2)The review notes that oral B12 is rarely prescribed outside Sweden and Canada, and that case-control and case-series studies had reported equal efficacy.
Vitamin B12 review summary, American Family Physician 2006What the studies found
Vitamin B12 as cyanocobalamin is a standard supplement. The trials compared oral and injected B12 in people who were already deficient, not in people with normal levels.
What this does not say
The New England Journal of Medicine's clinical review describes B12 deficiency as common, often missed, and fully correctable with injections that bypass absorption problems.
A 2021 systematic review found B12 supplementation improves fatigue and cognitive symptoms mainly in people who were deficient; that is why your physician checks your level before prescribing.

If your B12 is low or borderline, yes: fatigue, brain fog and tingling improve as levels recover, often within weeks. If your level is normal, extra B12 does not add energy. That is why we ask about your history and, when needed, check a level before prescribing.
Usually into the muscle of the upper arm or thigh, or under the skin of the abdomen, once a week to start; your physician shows you and sets the schedule. The kit includes everything.
Mostly mild: soreness or a small bruise where you inject, occasional nausea or headache, and with larger choline doses a fishy odor. Serious reactions are rare. Read the safety information above and tell your physician about every medication.
For B12, often yes: a level tells your physician whether you are deficient and how often you need injections. For lipotropic blends alongside a weight-loss plan, your physician decides from your history. Tests can be arranged locally.
