Important safety information and common side effects
Tirzepatide can cause nausea, vomiting, diarrhea, constipation, stomach pain, reduced appetite, fatigue and injection-site reactions, most often in the first weeks and when the dose goes up. Stop and seek care for severe stomach pain (possible pancreatitis or gallbladder problems), signs of an allergic reaction, or vision changes if you have diabetes. GLP-1 medications carry a boxed warning about thyroid C-cell tumors seen in rodents: do not use if you or a family member has had medullary thyroid carcinoma or MEN 2. Not for use in pregnancy or breastfeeding, or with a history of pancreatitis. Tell your physician about diabetes medication, insulin and every medication you take.
The research behind
tirzepatide.
Tirzepatide acts on two gut hormone receptors, GLP-1 and GIP. Its evidence is large randomized trials in adults with obesity, all with the injected form.
About the added ingredient: Vitamin B12 is added to many compounded blends. No trial has tested it for weight loss with tirzepatide.

SURMOUNT-1: 2,539 adults with obesity, or overweight with a weight-related condition, 72 weeks of weekly injections at 5, 10 or 15 mg. At 15 mg the mean loss was 20.9% (intention to treat), against 3.1% on placebo.
SURMOUNT-1, NEJM 2022 - Head to head20.2% vs 13.7%
SURMOUNT-5: 751 adults without diabetes, open label, 72 weeks. Maximum tolerated tirzepatide (10 or 15 mg) lost 20.2% on average; maximum tolerated semaglutide (1.7 or 2.4 mg) lost 13.7%.

SURMOUNT-1 at 15 mg: 57% of people reached a loss of at least 20% of body weight, against 3% on placebo. The average hides a wide spread between people.

What the studies found
- Average weight loss of 20.9% at 72 weeks on the 15 mg dose, against 3.1% on placebo (SURMOUNT-1, NEJM 2022).
- Lost more weight than semaglutide in a head-to-head trial: -20.2% against -13.7% (SURMOUNT-5, NEJM 2025).
- Reduced apnea-hypopnea events per hour by 20 to 24 more than placebo over 52 weeks in adults with obesity and sleep apnea (SURMOUNT-OSA, NEJM 2024).
- Improved all prespecified cardiometabolic measures alongside weight loss (SURMOUNT-1, NEJM 2022).
- Kept most of the weight loss while continued: 89.5% kept at least 80% of their loss, against 16.6% on placebo (SURMOUNT-4, JAMA 2024).
Trial results are for the injected tirzepatide at the doses studied. They are averages, not predictions for one person. No trial has tested the sublingual (dissolve-under-the-tongue) form or the compounded blends.
What this does not say
- SURMOUNT-5 was open label and run by the manufacturer of tirzepatide, so the head-to-head result needs an independent check.
- The trials ran 52 to 72 weeks. Effects after stopping were not part of them, though SURMOUNT-4 shows regain after withdrawal.
- Between 4.3% and 7.1% of people on tirzepatide stopped for adverse events in SURMOUNT-1, against 2.6% on placebo.
