What is ezetimibe?

(zetia, non-statin cholesterol lowering)

Ezetimibe blocks NPC1L1, a cholesterol transporter in your intestinal wall. This prevents dietary cholesterol (which you ingest and which your body makes and recycles) from entering the bloodstream. Less intestinal cholesterol absorption means lower LDL. It does not inhibit cholesterol synthesis the way statins do; it stops it from being absorbed. This makes it an ideal partner for statin therapy or for people who cannot tolerate statins.

Ezetimibe is very well tolerated. Side effects are rare; the most common are mild headache and GI upset. Do not use ezetimibe if you have active liver disease or if you are taking a statin and have elevated liver enzymes. Tell your physician about all medications and any history of muscle pain or weakness (myopathy risk is higher when ezetimibe is combined with a statin). Stop and seek care for severe muscle pain, dark urine, or signs of liver damage (jaundice, abdominal pain, nausea).
