Ezetimibe works in the gut, reducing how much dietary and biliary cholesterol the body absorbs. It is usually added to a statin. The question is not whether it lowers LDL, which it does, but whether lowering it further on top of a statin reduces heart events, and in whom.
The trial that tested heart events
IMPROVE-IT randomized 18,144 patients hospitalized with an acute coronary syndrome in the previous 10 days, with LDL cholesterol between 50 and 100 mg/dL, to simvastatin with ezetimibe or simvastatin alone. The primary composite was cardiovascular death, heart attack, stroke, hospital readmission for unstable angina, or revascularization. The authors reported a small but significant reduction in the composite on the combination over about seven years.
LDL on top of a statin
The combination lowered LDL cholesterol further than simvastatin alone at one year. A pooled meta-analysis of simvastatin trials found the same direction: a larger LDL reduction with the combination. The size of the LDL effect is what the trial was built to test, but the outcome benefit was modest.
Guideline position
The 2019 ESC/EAS guideline on dyslipidaemias places ezetimibe as an add-on to statins when LDL cholesterol remains above the target set for a person's risk. It does not present ezetimibe as a replacement for statin therapy.
Safety
The trial reported no significant excess of adverse events on the combination, and no signal of cancer harm in the summaries we reviewed. The trial was funded by the manufacturer of the combination tablet, which is one reason to read its results as a single trial rather than a settled rule.
What this does not say
- The benefit was shown after an acute coronary syndrome. It does not show that ezetimibe prevents heart events in people without known heart disease.
- The trial product was ezetimibe with simvastatin. Ezetimibe alone has no outcome trial of its own.
- The absolute event reduction over seven years was modest, and it depends on baseline risk.
How we use this
Our physicians add ezetimibe when LDL cholesterol stays above the target for your risk on a statin, and when side effects or other reasons make a higher-intensity statin a poor fit. We discuss the modest event benefit honestly, not as a guarantee.
Your LDL target and your risk are set together with you; we check the blood tests at the schedule the guideline recommends.
