How tirzepatide works
Two hormones, not one
Tirzepatide acts on the receptors of two gut hormones, GLP-1 and GIP. Semaglutide acts only on GLP-1.

It slows how fast the stomach empties and signals fullness to the brain, so smaller meals feel like enough.
It steps up slowly
The weekly dose starts at 2.5 mg and rises every 4 weeks, as your physician decides, up to 15 mg.
What the trials showed
In SURMOUNT-1, 2,539 adults with obesity or overweight took weekly tirzepatide or placebo for 72 weeks, with diet and exercise.
In SURMOUNT-5, tirzepatide led to 20.2% average weight loss at 72 weeks, against 13.7% with semaglutide.
These trials used brand-name tirzepatide. Compounded tirzepatide was not studied in them, and individual results vary.

Side effects and safety

- Decreased appetite, indigestion and stomach pain
- Injection-site reactions
Less common but serious
- Pancreatitis
- Gallbladder problems
- Low blood sugar, especially with insulin or sulfonylureas
- Kidney problems from dehydration
Not for you if
- You or your family have had medullary thyroid cancer or MEN 2 (thyroid C-cell tumors were seen in rodents)
- You are pregnant, trying to get pregnant or breastfeeding
- You have had pancreatitis

How much weight can I lose with tirzepatide?
In the SURMOUNT-1 trial, adults on 15 mg lost 20.9% of their body weight on average at 72 weeks, against 3.1% with placebo. Results vary from person to person, and compounded tirzepatide was not studied in that trial.
