Physician-prescribed hormone therapy

Estradiol cypionate for women in menopause or perimenopause and for gender-affirming care; gonadorelin for men who want to preserve their own testosterone signal, especially on testosterone therapy.

Symptoms have causes, and causes have treatments

Hot flashes, broken sleep, low mood, brain fog, joint aches: for many women these are the sound of estrogen falling. For men on testosterone therapy, a shrinking signal from the brain to the testes is what quietly costs fertility and testicular volume.

Both have well-studied treatments. What matters is the right candidate, the right dose and a physician who follows you.

  • Estradiol

Hormone therapy is the most effective treatment for hot flashes and night sweats, and it protects bone. For healthy women under 60 or within 10 years of menopause, the benefits generally outweigh the risks.

  • Gonadorelin

Gonadorelin is a copy of the brain's own GnRH signal. Given in small pulses it keeps the testes stimulated, which is why it is used alongside testosterone therapy and in fertility care.

  • Followed, not just prescribed

Hormones are adjusted, not set. Your physician reviews symptoms and, when useful, lab work at each check-in and changes the dose with you.

Disclaimer

Individual results vary. Statements about menopausal hormone therapy reflect the 2022 position statement of The North American Menopause Society; compounded estradiol and gonadorelin are not the FDA-approved brand-name products.

What to expect, month by month

  1. Day 1

A licensed physician reviews your history and, when needed, asks for labs before deciding on therapy and a starting dose.

  2. Weeks 2–4

Estradiol: hot flashes and night sweats often ease within the first weeks. Gonadorelin: the pulsed signal starts reaching the testes.

  3. 3 Months

Your physician reviews symptoms and any labs and adjusts the dose. Sleep, mood and energy usually settle by now.

  4. 6 Months

A stable dose. For gonadorelin on testosterone therapy, this is when preserved testicular volume and fertility markers are typically reassessed.

  5. 1 Year and Beyond

Ongoing therapy is reviewed at least yearly: the goal is the lowest dose that keeps you well, for as long as it makes sense for you.

Who hormone therapy is for, and who it isn't

Estrogen is not for everyone: a history of breast or uterine cancer, blood clots, stroke or unexplained bleeding rules it out, and the balance of benefit and risk changes with age and time since menopause.

That is exactly why it is prescribed after a real review of your history, not sold off a shelf. If it isn't right for you, your physician says so and helps you find what is.
