(bremelanotide; melanocortin receptor agonist)
PT-141 (bremelanotide) is a melanocortin receptor agonist that increases sexual interest and arousal by activating receptors in the brain and body. It is FDA-approved as Vyleesi for women with hypoactive sexual desire disorder (HSDD). It is used off-label in men with low desire. Given as a subcutaneous injection 2–4 hours before sexual activity, PT-141 does not require daily dosing. Common side effect: nausea in the first hours (30–50% of users), often mild but sometimes requiring an antiemetic.
FDA-approved
for women with HSDD (Vyleesi); off-label use in men
PT-141 works for sexual interest; arousal and response still depend on desire and stimulation. It is not prescribed with uncontrolled high blood pressure (melanocortin agonists can raise BP). Nausea is common but usually mild.
Adults whose problem is desire, not blood flow.
It acts on desire in the brain. FDA-approved, as Vyleesi, for premenopausal women.
Nausea and flushing are common with the first doses. No more than one dose in 24 hours.

dermatology for Melanotan II, BP for PT-141
Day 1
A US-licensed physician reviews your goals, medical history, and medications. For Melanotan II, a dermatologist clears your skin. For PT-141, your BP and cardiac history are checked.
1Week 1
For Melanotan II: small doses start the tanning process. For PT-141: first dose is tested privately to confirm tolerance and response.
2Week 2–4
Melanotan II: tan develops gradually over weeks. PT-141: sexual interest and arousal increase predictably on-demand.
3Month 2
Your physician checks in: Melanotan II users report on tanning progress and any mole changes (dermatology follow-up required). PT-141 users discuss sexual satisfaction and any side effects.
4Beyond
Melanotan II: ongoing dermatology checks are non-negotiable. PT-141: use as-needed; your physician reviews annually or if changes occur.
5Individual results vary. Benefits reflect what Zepym Health members commonly report to their physician at check-ins; they are not the results of a clinical trial.

A sample week
Once a day
In trials of women with HSDD, PT-141 significantly increased sexual desire, arousal, and satisfaction compared to placebo. About 40% of women showed meaningful benefit.
Off-label use in men for low desire is growing; evidence is anecdotal and from small studies. Your physician discusses realistic expectations: PT-141 does not overcome relationship problems or performance anxiety alone.
Learn moreMelanotan II and PT-141 activate melanocortin receptors throughout the body, with different safety profiles. Melanotan II: melanoma risk is the primary concern; baseline skin exam with dermoscopy and regular mole checks are mandatory. Darkening of existing moles can make monitoring harder. PT-141: nausea is common (30–50%), usually mild; do not use with uncontrolled high blood pressure, recent MI, unstable angina, or severe heart disease. In men, priapism (erection lasting >4 hours) is rare but requires immediate care. Tell your physician about blood pressure medications, heart history, and any skin concerns. Stop and seek care for severe headache, chest pain, priapism, severe nausea, or signs of allergic reaction.

Injected under the skin, once a day. The starting prescription is 10 units (0.2 mg) each time. Your physician sets your own dose and it is printed on your label.
1 vial at the starting strength, shipped to your door from a licensed US pharmacy.
Inject subcutaneously 2–4 hours before anticipated sexual activity. Effects peak around 1–2 hours after injection and last several hours. One dose per 24 hours maximum. If nausea occurs, taking an anti-nausea medication (like ondansetron) 30 minutes before PT-141 helps some people. Eat a light meal beforehand; a full meal can delay effects.
Melanotan II: tan develops over 1–4 weeks of consistent dosing; deepens over months. PT-141: sexual effects appear within 1–2 hours of injection, peak around 2 hours, last several hours. Both require consistent use or dosing to maintain results.
Melanotan II: darkening of moles, nausea, or erections in some men. Report all mole changes to your dermatologist immediately. PT-141: nausea is common (30–50%); anti-nausea medication beforehand helps. Rare: priapism in men (erection >4 hours)—seek emergency care. Stop immediately if any severe reaction occurs.
Tell your physician about all medications, especially blood pressure drugs or cardiac medications. Some combinations need monitoring. Do not use Melanotan II with a personal or family history of melanoma without careful discussion with dermatology.
Melanotan II: as long as you want the tan; dermatology checks remain mandatory. PT-141: as-needed indefinitely, or until your sexual health goals change. Annual check-ins with your physician are recommended.
